  • Call U-Connect - 770.232.5224
  • Provide name, address, SSN, Mediacaid #, DOB
  • This is a free service for those who are 65 and older and/or disabled.
  • Receiving Medicaid and SSI.
  • Medical conditions such as diabetes, arthritis, Alzheimer's disease, dementia, heart disease or stroke that require monitoring and regular doctor's care, or interfere with self.

 

In-home services:

  • Coordination of home health
  • Personal support
  • Homemaking
  • Home-delivered meals
  • Emergency response
  • Caregiver respite
  • Coordination of transportation
  • Errands

 

Community services:

  • Adult daycare
  • Personal care home
  • Caregiver respite
  • Coordination of outpatient therapy

 

No waiting list and they will refer client to other agencies if they don't qualify for income restriction for Source.
